- MED 2375: Reported to Medical for issues of dehydration and exhaustion after Breen Attack.
- CNS 2375: Officer reported to discuss feelings of isolation and issues with a superior officer. No further counseling requested.
- FLEET 2376: Graduates Starfleet Academy.
- FLEET 2376: Posted to Seventh Fleet, Courier Pilot.
- GEN 2376: Decoration given by Starfleet on 55327.8: Ribbon of Sacrifice

Adding Information After Approval
It is important to remember that your service record must be kept up to date! The SR is one of the tools used by Command Staff and Department Heads in determining promotion, eligibility for away missions, and other IC events. One of the main sources for new entries will be episodes, but there can, and will, be other times information needs to be added. Below is an example of how to add information to your SR after approval.
+sr/add 2377=Participates in Operation Homecoming under the command of Captain Mitellan as Operations Officer. USS Marathon dispatched to the Wormhole to rescue a small ship piloted by Bajoran youths which had lost power. Instrumental in providing informational support.
Departmental Additions
There are several departments which have the ability to add to the SR of another officer. Command, Medical, Counseling, Security Officers and the CO can add entries under the appropriate heading. If you are an officer in one of these departments and need to make an entry on another officer's SR, simply type +<department abbreviation>/add <name>=<text>. Below is an example of how a medical officer can add information to an SR.
+med/add Mitellan=Suffered a broken femur and fractured tibia as the result of a shuttle craft accident. Treated with autosuture and restricted to light duty for 96 hours. Officer advised to report to medical in 72 hours for follow-up.
